Must show proof of logged dives within the last 12 months
Must check in at the dive center the day before the dive trip for equipment sizing and registration
All divers are required to complete the Health Declaration Form Must provide a letter of consent from a physician if any answer on the Health Declaration is a YES.
All divers over the age of 55 must present a letter of consent from a physician permitting the diver to partake in diving activities.
Need to pay the national park fees: Day 1 600 baht Day 2: 200 baht If adding additional days diving: day 3 and day 4 is 300 baht each day
